Through strong relationship building with collaterals and staff, the Grant Program Coordinator/Evaluator will manage all aspects of grant coordination and evaluation in alignment with specific requirements of each grant. The Grant Program Coordinator/Evaluator will oversee all logistical aspects of grant programs, under the supervision of the Choices Program Director and The Grant Project Director. The Grant Program Coordinator/Evaluator will handle day to day operations, schedule coordination, facilitate marketing and outreach, manage partnership engagement, troubleshoot program challenges as they arise, complete and submit invoicing for approval and conduct frontline budget management. The Program Coordinator will manage and report on all aspects of grant data requirements and provide quarterly progress reports to the Program/Project Director. This position will provide behavioral health services and case management to adolescents as part of the multi-disciplinary CMCA team. Services include providing assessments of youth affected by substance use, coordination of care, case management, treatment of substance use disorders and co-occurring mental health disorders, and referrals to higher levels of care if clinically indicated.  Download 

Through strong relationship building with collaterals and staff, the Program Manager will manage all

aspects of diversion programming, including direct supervision and evaluation of grant employees. The Program Manager will report to the Project Director. The Program Manager will lead day to day operations of the Diversion Program, including schedule coordination in partnership with the case manager and clinician, marketing and outreach, partnership and family engagement, troubleshooting program challenges as they arise, assignment and accountability of staff, and all aspects of enrollment, attendance and engagement of adolescents and families. The Program Manager will manage and report on all aspects of grant data reporting requirements, and provide quarterly progress reports to the Grant Project Director. Download

Through strong relationship building with collaterals and staff and positive interactions with adolescents and young adults in the program, the Choices Social Worker will provide behavioral health services and case management to adolescents as part of the multi-disciplinary Choices Team. Services include providing assessments of youth affected by substance use, coordination of care, case management, treatment of substance use disorders and co-occurring mental health disorders, and referrals to higher levels of care. 

Duties also include, but are not limited to, conducting psychosocial assessments, making a psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, coaching and mentoring of the adolescent and family members and crisis intervention. This position collaborates and communicates frequently with a wide range of government agencies, courts, schools, clinics, and hospitals to assist in effective care of referred adolescents.  Download

Through strong relationship building with collaterals and staff and positive interactions with adolescents and young adults in the community, the case worker will assess a client’s current level of stability, and in collaboration with team members, create an action plan for intervention and support. Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an adolescent’s health and human services needs.
